- Consultation with a care manager
answers questions and provides information on care options, living
situations ad community resources.
- In-Home Assessments evaluate the
older adult’s ability to remain safely in the home environment. The
evaluation includes an assessment of the mental, physical and
emotional status of the older adult and the identification of
available support systems including community resources to promote
independent living.
We will also
help determine their need for long-term care facilities.
- Independent Living Plans are
developed to maintain the client’s safety and security and assure
the older adult and caregiver of the capacity to maintain
independence.
- In-Home Monitoring is offered to
assess safety, appraise levels of interventions and contact
additional community resources if necessary. Findings are
communicated with the client and family.
- Housekeeping Services provide
general cleaning, laundry and meal preparation to promote safety and
well-being in the home.

Counseling – Out patient sessions are provided by licensed clinical
social workers in both our Wilmington and Newark locations. Most
insurances, including Medicare are accepted.
Psychiatric assessments and medication checks are provided by
board certified psychiatrists. Most insurances are
accepted.
Care Management
fees are not covered by insurances. Fees are dependent upon the specific
service and level of care requested.

Delaware Aging Network
The
Delaware Aging Network
(DAN), with
initial pilot project funding from the Delaware Community Foundation,
is a statewide coalition of more than 21 nonprofit organizations, whose
purpose is to increase efficiency, reduce duplication of services, and
assure that older adults will receive the care they need to remain
independent. This integrated, collaborative network of service providers
expands outreach to and services for the older adult population, while
addressing the interrelated concerns of housing, health and
transportation. By creating this system of care, older adults will meet
the challenges of growing older.
Jewish Family
Services was selected by the coalition as the lead agency for New Castle
County. The Modern Maturity Center serves Kent County; Cheers serves
Sussex County.
