Finding A Way Home

The Finding A Way Home program assists adults with complex health care needs who choose to move from a nursing facility to live with family members or to their own independent setting. With assistance from a Transition Coordinator, eligible consumers develop their own Transition Plan and may receive a wide range of services including personal care, personal attendant services, home modifications, respite care, and one-time household set-up expenses.

 
 
 

  

To be eligible, an individual must:

  • Be a resident of a nursing facility for at least 6 months
  • Be eligible for Medicaid
  • Have healthcare needs that can be safely met in a community setting
  • Be eligible for one of Delaware’s Medicaid Waiver programs, and
  • Consent to move back to a family home or to an apartment with an individual lease.
