JFS Care Navigator

TWO POSITIONS: seeking applicants for Wilmington & Sussex

JFS is looking for a Care Navigator to join the new Care Navigation program, designed to support the needs of older adults who want to age safely and with dignity in their own homes or their community of choice. Care Navigators combine compassionate care with the expertise necessary to help older adults and/or their caregivers manage the growing and complex responsibilities of aging. Services are tailored to meet the individual needs of clients and provide peace of mind for them and their families.

Essential Responsibilities Include:

  • In-Home Assessment
    & Development of Individualized Care Plan
  • Care Management/Coordination
  • Wellness Monitoring
  • Patient/Medical Advocacy
  • Document Preparation (e.g. benefits applications)
  • Providing transportation may be required for medical appointment accompaniment

This position includes meeting people in their homes or at other community locations. The ideal candidate has strong engagement and interpersonal skills, and is comfortable making home visits and meeting with families/caregivers on an as-needed or on-call basis.

BSW, BSN, MSW and/or experience working with older adults and caregivers preferred. Driver’s license & clean driving record, proof of insurance, and car in good working condition required.

This is a part-time position. Hours must be flexible an may include weekends or evenings.

Agency Administrator

Wilmington, Full-Time

This full-time position provides business management of the agency’s programs and services. Working in partnership with the Senior Management Team, the Agency Administrator ensures that the services we offer are financially sustainable, reflective of community need, and operating efficiently. The Agency Administrator will ensure our services are delivered with the highest efficiency, quality, and standard by monitoring our performance against each contract, our budget, and our strategic growth goals. This position plays a leading role in managing both systems and personnel. Oversight of inventory, purchasing, and supplies is central to the job. Human resources tasks include determining needs, coordinating hiring, setting training standards and hiring procedures. The Agency Administrator will also analyze and improve organizational processes and improve information management systems.

This role will not serve in a supervisory capacity; instead, the person in this role will be looking very closely at profit margins and untapped opportunities. The Agency Administrator will work with senior management to further develop existing business opportunities such as growth potential for Care Navigation and Counseling.

At least 5 years of related experience is preferred.
