Licensed Care Navigation Team Provides Personalized Support

The JFS Care Navigation Team is comprised of licensed clinical social workers and other experienced professionals who provide comprehensive assessments, in-home counseling, patient advocacy, and support.

With personalized short-term solutions and long-range plans, we improve and foster safety and independence to promote a higher quality of life.

Meeting with one of JFS’ Care Navigators is an opportunity to discuss challenges, opportunities, and strategies for incorporating needed services that help make aging more manageable.

Whether you are local or trying to manage the special challenges of caring for your loved one from a distance, JFS can help you navigate the available resources and benefits.

We work to identify appropriate options and help the family select the course of action that will ensure the well-being of loved ones, as well as peace of mind for caregivers.

In-Home Assessment & Personalized Care Plan

Through thoughtful questioning and discussion, a Personalized Care Plan is developed and implemented to improve and foster safety, independence and to enhance overall well-being.

The JFS Care Navigator meets with our clients, their families, and other concerned parties to assess all aspects of the older adult client’s life, including general health, medical treatments and providers, medications, mood, cognitive ability, physical ability, nutrition, home safety, social supports, current benefits and relevant legal issues.

Care Management / Coordination

Give older adults access to the help they need through a consistent relationship with an experienced and compassionate JFS Care Navigator.

This relationship ensures that a professional is available to monitor and address changing needs, as well as to address any crisis that may occur.

Once the Personalized Care Plan is defined and implemented, home visits and frequent phone contact allow JFS Care Navigators to evaluate services and suggest changes when appropriate. A designated JFS Care Navigator has the experience to:

  • Find, interview, and manage homecare services
  • Follow through on housing, from application to settling in
  • Make connections to community resources
  • Advocate for the client and report back to family members
  • Coordinate/facilitate appropriate medical care
  • Monitor ongoing effectiveness of the care plan

Medical Advocacy

As an extension of your own family, our JFS Care Navigators advocate and mediate on behalf of our clients, serving as an additional set of eyes and ears for medical issues.

The JFS Care Navigator can accompany the client to health care appointments and facilitate communication with the provider to ensure that concerns are expressed, and medical issues are understood.

For caregivers, this service greatly eases the stress and responsibility of staying up to date on the medical care needs of their loved ones.

Wellness Monitoring

JFS conducts in-home visits as often as recommended or requested, giving you peace of mind.

During these visits, the JFS Care Navigator meets with the older adults and their families to evaluate well-being and unmet safety concerns.

Home visits and frequent phone contact allow JFS Care Navigators to continuously track how well clients are doing, to evaluate whether current services are meeting needs, and to implement changes in a timely manner.

Therapeutic Counseling

Whether facing challenges in everyday life or times of crisis, JFS’ licensed, caring, and experienced therapists are ready to help.

JFS therapists provide strategies for coping with depression, anxiety, the challenges of chronic illness, and end of life issues, including family and relationship challenges by helping to identify, build, and utilize one’s own strengths and skills.

Through support and guidance, we help you gain a clearer understanding of your situation and work with you to create an individualized plan specific to your needs.

What is a Care Navigator?

A Care Navigator, also known as a care manager, is a professional who specializes in assisting older adults and their families in meeting long-term care arrangements. JFS’ Care Navigators are all master’s level mental health professionals, social workers and registered nurses, with extensive experience working with older adults and their families. Many of our professionals are also licensed as clinical social workers.

When is a Care Navigator needed?

A Care Navigator is needed when an adult is faced with decreased capacity to function due to complications of aging, illness, disability, altered cognitive status and/or loss of social supports. Care Navigators provide caregivers with assistance, especially when they are out-of-state and not available to provide direct care because of family and other obligations.  Assistance with decision-making, issues with problem solving, family conflict, relocation, hospitalizations and monitoring for significant changes are often the reasons that families initiate Care Navigation services.

How does it work?

Care Navigators engage in an assessment process, develop an individualized care plan, and offer immediate support and options for care, with a focus on long-term needs.

The qualified Care Navigator can coordinate services such as meals, transportation, pet care, home maintenance, assistance in hiring providers for home care, medication management, applying for benefits, as well as accompany the adult to medical appointments.

The professional monitors health and providers of care towards enhancing the adults’ independence and autonomy; encouraging their ability to remain in their own homes.

Why would I use a Care Navigator?

Should you have concerns about a frail or disabled adult, or need information and assistance to ensure safety, a Care Navigator can be a valued contact.

Professionals are not a substitute for family, but as the local “eyes and ears”, they can be relied upon to help identify service needs, advocate for your older relative, and be a liaison to other service providers.

Having a Care Navigator can be an especially important mechanism to establish that a “safety net” for elders is in place, should a crisis occur.

How does one pay for Care Navigation services?

Care Navigation services are usually paid for privately by family members, through an hourly fee-for-service arrangement. Fees are stated in written form, and are documented in a written agreement.

Medicare and Medicaid cover therapeutic counseling but do not cover case management services; they are paid for privately by family or other caregivers.

JFS will bill insurance whenever possible. JFS offers an easy reoccurring credit card payment option. JFS also accepts personal checks and cash.

How can JFS professional Care Navigators save you money?

Planning for the future and averting a crisis are just a couple of the ways that Care Navigation can help save you money.

It can be more cost effective to hire a professional than to try coordinating services alone. Our Care Navigators have a wealth of knowledge and existing relationships with other healthcare providers in the community, which allow them to efficiently arrange for needed assistance.

Having a professional “on the ground” to respond to crises may also mean that caregivers can plan their trips to visit, rather than paying exorbitant prices to travel when an emergency takes place.

How can you evaluate the Care Navigator?

Discussing your expectations is a good place to start to gain a solid understanding of how the services will meet your needs.  Some questions to ask include:

  • What type of oversight is there for the Care Navigator (who supervises the Care Navigator)?
  • What accessibility will I have to the Care Navigator on weekends and evenings?
  • Does the service feel personalized to meet the unique desires and needs of my family?
  • Is the Care Navigator responsive to my concerns?
  • Does the Care Navigator stay on top of things?
  • Are the other service providers with whom the Care Navigator coordinates meeting the need of my older relative?
  • How does the Care Navigator monitor the quality of the providers?
  • Is the Care Navigator pro-active, ahead of the crisis?

With written permission from the client, JFS regularly communicates with family members to provide updates, answer questions and provide peace of mind.
