DEMENTIA CARE & CAREGIVER SUPPORT

JFS COMPASS, which stands for Caring for Older adults with Memory Problems And their Support Systems, is an interdisciplinary team of geriatric professionals who provide individually-tailored, hands-on outpatient medical care and care management to older adults with dementia, and critical support to their caregivers.

Through education, counseling, and improved access to services, COMPASS provides comprehensive assessment, diagnosis, treatment of symptoms, and ongoing support to caregivers during all stages of the disease. Together, we work with you and your loved one to develop a personalized plan that offers strategies to address day-to-day issues while promoting well-being.

  • Who We Serve:

    • Individuals with dementia AND their caregivers; both enroll in the program
    • Patients who already have a primary care provider
    • Individuals who live at home or in a facility
    • Patients from all over Delaware and the surrounding region, including Pennsylvania
    • Patients referred from family members or professionals
    • Patients who can be accompanied by at least one caregiver for all appointments (this is a MUST); additional caregivers and family members are encouraged to participate
